As a Sr. Clinical Pharmacist, you will build and maintain relationships with provider groups and pharmacies to drive improvement in Part D quality metrics. The Sr. Clinical Pharmacist will conduct research and maintain or enhance clinical pharmacy programs such as drug therapy monitoring, health outcomes, disease state management, and care management. Employees in this position will work as a critical part of the Quality Field Operations team; aligning to geographical regions and clinics that care for a high volume of Medicare membership. The Sr. Clinical Pharmacist is responsible for supporting care management related pharmacy activities, drug therapy monitoring, and staff education, with a focus on improved health outcomes.

Primary Responsibilities:
- Develop and implement Part D strategy across assigned territory
- Collaborate with an integrated health care delivery team for pharmacy related practice quality improvement
- Counsel providers and pharmacies regarding Part D quality measures and clinical guidelines
- Conduct practice specific pharmacy data review to improve adherence to specified medications
- Be available and act as a clinical/pharmacy resource for the field-based teams
- Proactively identify performance improvement opportunities through comprehensive data analysis
- Provide education about medication adherence barriers and maximizes members' medication success through improving or maintaining adherence to medications
- Coordinate and support strategic member outreach campaigns
- Deliver high quality evidence-based therapeutic recommendations to providers
- Provide appropriate and effective education, coaching, and consultation for providers, support staff, and internal field-based teams
- Effectively participate in and contribute to staff meetings and other staff development opportunities and interdepartmental work groups
- Identify areas of improvement where systems modifications could result in operational advancements and efficiencies
- Maintain a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as an authoritative information source on drugs and their utilization in therapy
- Assist with hiring support staff that will help with pharmacy initiatives, as required

Required Qualifications:
- Pharmacy Degree with an active, unrestricted license to practice as a Pharmacist in the State of Indiana
- 5+ years of experience as licensed Pharmacist
- 2+ years as a team lead and taking the lead on direction for support staff
- HEDIS and CMS star rating experience
- Experience providing clinical pharmacy services to Medicare patients (especially Parts C and D)
- Experience collaborating with and presenting to senior leadership and stakeholders
- Proficient using MS Office Tools, specifically Excel (creating and executing reports is required) and PowerPoint (peer-to-peer presentation and education required)
- Reliable vehicle, current and unrestricted driver’s license, and proof of auto insurance
- Must be willing/able to travel approximately 50% of the time as business needs dictate (the state of Indiana)
- Must live in the State of Indiana to perform the functions of this role (routine travel is required within the state)

Preferred Qualifications:
- Pharm D
- MTM (Medication Therapy Management) Certification
- Current Geriatric Licensure or in process of obtaining: CGP (Certified Geriatric Pharmacist)
- BCPS or BCACP: Board Certified Pharmacotherapy Specialist or Board Certified Ambulatory Care Pharmacist
- Health Plan (Managed Care) experience
