SCOPE OF RESPONSIBILITY
Manages and implements on-site quality programs, and quality improvement initiatives.
Maintains the accreditation process and responsible for monitoring and quarterly auditing to be in alignment with continuous state of readiness for AAAHC Accreditation and HFAP.
Oversees relationship and preparation for survey process and on-site visits from regulatory and accrediting agencies including planning measures that minimize risk levels within the organization, identifies and corrects deficiencies as they relate to remaining in continuous compliance with the standards of AAAHC, HFAP, and healthcare accrediting bodies.
Supports all Cummins/Premise Health sites with regard to the company quality programs and initiatives.
PURPOSE OF THE POSITION
The Quality & Accreditation Supervisor will report directly to the DCO, Onsite at the Cummins LiveWell Center, and will provide quality support and offer guidance for building and maintaining continuous quality improvement programs that adhere to regulatory, compliance, accreditation, and legal concerns while providing a safe and effective program that helps our patients get, stay and be well.
ESSENTIAL JOB FUNCTIONS
Lead the Quality Program at Cummins LiveWell Center and other Cummins/Premise Health sites.
Execute and manage training programs on quality concepts and activities.
Manage and coordinate accreditation activities to assure continued Network Accreditation.
Provide assistance in educating leadership, physicians, and staff on highly reliable methods for identifying and monitoring potential high risk problem-prone areas and setting up quality activities for study utilizing evidenced based, industry standards as benchmarks.
Assist Medical Directors, Operations Leadership, and site leaders with planning and integrating functions and services to support Premise Health clinical priorities and goals for clinical quality improvement activities.
Lead teams in performance improvement activities utilizing root cause analysis as needed.
Build effective working relationships throughout the organization providing collaboration on activities that focus on improved employee training, education, employee retention, clinical effectiveness and workflow efficiencies.
Identify opportunities for improvement based on evidence based practices, performance guarantees and activities established by the teams with the client.
Communicate aggregated patient satisfaction results and collaborate with colleagues to improve processes utilizing the tools built for improvement.
Assist site leadership with access and interpretation of monthly reports for various purposes including department updates, quality reports, evidence based care, etc. as requested.
Manage key platforms for presentation and housing of quality management material: Excel, Power point, Word, share point, QI database.
Maintain a knowledge of industry standards, regulations, and application of the standards applicable to company lines of service and quality improvement practices and processes.
Educate site leadership in the assessment of intent and applicability of regulations and standards.
Engage appropriate key stakeholders in the design, implementation, and monitoring of planned improvements to ensure compliance and sustainability of key site activities.
Manage survey processes.
Manage Quality Program for collection, analysis and reporting of quality activities promoting interdisciplinary collaboration that fosters team work and team engagement.
Follow the Premise Health program for Continuous Quality Improvement which involves assessment of AAAHC and HFAP standard requirements and building a plan of action in collaboration with both operations and medical operations leaders to align sites for success in achievement of Network accreditation or re-accreditation.
Oversee Occurrence Report Process
Bachelor's Degree in Healthcare or related field strongly preferred.
Prefer experience in health care quality education and management, infection control and prevention, risk management, safety, Behavioral Health management, regulatory compliance, accreditation.
Demonstrates knowledge of state and other applicable regulatory requirements.
Possess in depth knowledge of site operation functions and quality improvement requirements as outlined by AAAHC, the state of employment's Department of Health and Federal regulatory standards (CMS), OSHA, CDC, WHO.
Strong organizational, prioritizing and analytical skills.
Ability to make independent decisions when circumstances warrant and lead a project to completion.
Knowledge regarding Risk Management systems and risk avoidance, variance reporting, participates and collaborates with colleagues to develop root cause analysis, failure mode effect analysis and investigate satisfaction/concerns as required.
Working experience applying concepts of process improvement for the purpose for coaching leaders in the identification and the elimination of waste by preventative action design, efficiency, effectiveness, and sustainability of quality management process.
Knowledge and Skills:
Exhibit positive interpersonal and communication skills and promotes effective interdepartmental relationships.
Manages time effectively to accomplish job responsibilities and take projects to completion.
User proficiency in Share point, Microsoft, Word, Excel, and PowerPoint.
Must demonstrate strong organizational and communication skills. Ability to work well under pressure and flexibility is a must.
Positive attitude, customer-service skills with a focus on availability and support to the customer.
This job operates in a clinic setting. This role requires (Insert details) the ability to travel if needed to other out of state Cummins locations. Travel would be very limited annually, but always a possibility if needed. This position will require up to 10% travel
